Address Change

To change your address, please fill out and submit the following forms to the staff members indicated.
W-4 - Return to Alicia Chidester
Local Earned Income Tax - Return to Jessica Worden
Name Change

Submit a copy of one of the following documents below to the Human Resources Department as proof of a name change:
- Marriage Certificate
- Updated Social Security Card
- Updated Driver’s License
- Court Order approving the change
In addition, submit the following updated documents:
- Local Earned Income Tax form
- W-4
- Updated Health, Dental and Eye forms if applicable
Professional Staff: Log in to your TIMS account and forward an updated copy of your teaching certificate to the HR Department.
Complete a Direct Deposit Authorization Form (found in the FIS Payroll System) and send it to Alicia Chidester, Payroll Coordinator.
If a change in marital status has occurred, consider updating the documents below:
- Benefit Election Form and Health, Dental and Vision Insurance Application (forms can be found on the website)
- Life Insurance Beneficiary (Form on website)
- PSERS Beneficiary Form
