• FMLA (Family Medical Leave Act) Request Procedures

    If you will be absent from your position for more than three days and have been in your position for more than one year, you must apply for a leave of absence, as required under the federal Family Medical Leave Act. 

    1.  Notify your principal or supervisor of your leave request
    2.  Notify HR in person, via email (jworden@mvsd.net) or by phone (570-434-8413) for the forms which are appropriate for your leave situation (It is imperative that you notify your supervisor and HR of your request).You can also find the forms by clicking the FMLA Forms link at the bottom on thsi page. 
    3.  Contact Alicia Chidester (achidester@mvsd.net), Coordinator of Payroll, if you have any questions regarding your pay while you are on leave.

    Intermittent Family Medical Leave Request:

          Payroll will enter your absences into the FIS system.  Please notify payroll either prior to or the day of using an Intermittent FMLA day. 

    Full-time Family Medical Leave Request:

          Once your request form is received and approved by the District Office, the payroll department will enter your absence dates into FIS with the correct FMLA code(s) (sick, personal, leave without pay) If you need to amend your dates or have a question, please contact Alicia Chidester, Coordinator of Payroll (achidester@mvsd.net) with all corrections.
